COMPEX Corporation Benefits Summary

COMPENSATION:
COMPEX’s policy is to offer competitive salaries and to reward good performance. An initial performance review is conducted after a ninety (90) day introductory period with regular performance and salary reviews beginning after one (1) year of service.

HEALTH INSURANCE:
COMPEX offers a contributory insurance package, which includes Medical, Dental, Vision, and Prescription Drug coverage. Employees will have a choice between two plans for Single and Family coverage, in which both plans are a Preferred Provider Organization (PPO) plan. For Single coverage you can choose the 80/60 plan which will be at no pre-tax deduction to employees; or the 100/80 plan which will be a pre-tax deduction of $22.50 per pay period. To add family to your plan the amount for the 80/60 plan would be $126.50 per pay period; for the 100/80 plan, the deduction would be $194.00 per pay period. If you wanted to get Dental coverage alone, then for single coverage it would be $16.00 per pay period, and for family it would be $37.50 per pay period.

SHORT & LONG TERM DISABILITY:
100% company paid short term disability (STD) benefits will cover 60% of your weekly earnings, up to a maximum of $750 per week. STD benefits will be paid for 13 weeks or until the employee can return to work. If the employee is still under a physician’s care after a 13-week period, long term disability (LTD) benefits will commence. 100% company paid LTD benefits will cover 60% of your monthly earnings up to a maximum of $10,000 per month. Payments will be made until age 65 or 67.

LIFE, ACCIDENTAL & DISMEMBERMENT INSURANCE:
100% Employer paid Life Insurance Benefit will provide 2 ½ times your salary up to $200,000. Accidental Benefits pay an additional 2 ½ times your salary up to $200,000 if any employee dies in an accident. Dismemberment coverage will provide 2 ½ times your salary up to $200,000. Accelerated Benefits Included: (this allows an Insured to collect 50% of Life and Accidental Benefit up to $100,000, if there is a life expectancy of 12 months or less).

SECTION 125 FLEXIBLE BENEFITS:
COMPEX also offers a flexible benefit program that includes pre-tax payroll deductions for dependent care and unreimbursed medical expenses. Optional insurance offered using pre-tax dollars include cancer, hospital intensive care, and accident/disability coverage. Group Term Life Insurance is offered as an after-tax deduction. These optional policies are portable.

LONG TERM CARE:
COMPEX offers Long Term Care (LTC) Insurance to all employees through Unum Provident. Compex will pay for a benefit of $1,000 per month for all eligible employees. LTC will be fully portable, which means that you may take the policy with you when you retire.

401K SAVINGS PLAN:
COMPEX matches employee, contributed funds at fifty percent (50%) up to the first six percent (6%) of the employee’s contribution. A statutory maximum of 60% may be contributed as a pre-tax deduction for federal and state income tax. This plan has a progressive three year vesting schedule (33 ?% vested after one (1) year, 66 ?% vested after two years, 100% vested after three years of service). Open enrollment is conducted each January and July.

EDUCATION REIMBURSEMENT:
COMPEX will reimburse individuals up to $3,000 per calendar year towards the cost of tuition, registration, lab fees, and books for completed courses eligible under COMPEX’s Tuition Reimbursement Policy. Approved course work under certification programs is reimbursable through this benefit.

ANNUAL LEAVE:
For the first year COMPEX provides fifteen (15) days of paid leave. The annual leave increases by one day per year for each year of employment completed based on the employer’s hire date.

PAID HOLIDAYS:
COMPEX offers ten paid holidays per year including Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, New Years Day, Martin Luther King, Jr. Day, President’s Day, Memorial Day, Independence Day, and Labor Day.

DIRECT DEPOSIT:
Direct deposit is effective on month after employment so those funds are available immediately on payday. One to five individual accounts may be selected.

EMPLOYEE ASSISTANCE PROGRAM:
COMPEX offers free, confidential counseling through UNICARE EAP for employees and their families.

ALEXANDRIA/SPRINGFIELD OLYMPUS SPORT:
COMPEX offers a reduced membership rate of $18.00 per month ($9.00 per pay period) that will be deducted from your paycheck semi-monthly. Employee family members can also join at this reduced rate although they will pay directly to Alexandria/Springfield Olympus Sport Club by the family member. The membership includes a fitness assessment that will assist in setting up individual exercise programs, free court time, free daycare, free aerobics, and full use of all cardio equipment and weight machines.
